As a DM who gave a unicorn familiar an aggressive lisp, I can safely say that you shouldn't give an NPC any kind of accent or speech quirk that you could grow to hate. 20 sessions of that familiar... and of COURSE the party kept asking her to do the "She sells seashells" tongue twister.
