Holtz ABSOLUTELY set that up! As he said - to ensure that Connor/Steven would go on loving him after he was gone. So he could be finished with revenge. He gave Angel the letter for Connor to make Angel look even more guilty. He implied in the letter that he trusted Angel with his son's well being, that he had moved on and maybe even forgiven... which then makes Angel attacking Holtz seem completely indefensible. He took Angel's son and raised him and parented him so that he could use his own death, which he asked Justine to help him with, as the final act to turn Connor against Angel and ensure he'd hate him forever for what he believed Angel had done.
