I love this episode, I always feel so bad for Buffy in this episode. The mention of Joss wanting to put a reference of scott summers having a cousin in a mental institution he means the Buffy in the institution we see in this episode. So the demon instead of sending her to a dream world instead made her see an alternate reality. In my own headcannon I think the Buffy in the institution is also a mutant who can see into alternate realities as her mutant ability but it's fairly indistinguishable from mental illness.
