when it comes to the GoT world, George RR Martin did base a lot of it around actual history and tried to keep it as close to it as possible, that's why there's also a lot sexual violence against women because that actually did happen during wars. but other than that, sexism of women in power (and women in general), homophobia and such were considered very taboo and a no-no back in the day of like kings and queens, but incest wasn't that talked about. it was expected to wed cousins and such to keep the 'royal bloodline' pure. Cleopatra married her brother i believe.
over time direct siblings were more of a no-no (like in Westeros, unless you're a Targaryen) but cousins were always fair game

you mentioned the whole back in the day when sex was just sex, you're talking about the Greeks actually, they were a lot more liberated about it but when the Roman empire came they actually scaled it down a bit, same-sex started getting taboo after it fell or during? can't remember but i do know the Greeks were a lot better at it
