It's a sim! Doing more areas at once is pretty problematic though, things can jiggle out of control. You'd need a proper muscle simulation for that. I know it's possible to do in Blender but the amount of skill and time to set this up properly is insane. Check out LustGard - it's a small team of people and I think they're the only ones that are doing muscle simulations in Blender (for lewd purposes). And check out Evalion - he recently started doing muscle simulations in Houdini and that's super insane in it's own right. He does mostly vore though.
