How to handle commissions?

So right now I'm basically doing one or two free commissions a month for everyone on this tier but just thinking ahead I can see this getting out of hand a few months from now. I want to get everyones opinion on how I should do them going forward. Some of you have suggested having slots which I think is a good idea but I want to make it fair for everyone without overwhelming myself (this isn't my day job although that would be so awesome haha). I had thoughts about setting up 5-10 slots and opening it up to anyone but everyone on the hero tier will get a slot for free where others will have to pay something for it.
Another question I have is what should distinguish the top tier from the other. I love doing commissions and getting ideas from everyone (btw everyones shared ideas have been AMAZING and I'm working on several of them right now). Should the top tier only be allowed commissions? Or should I open it up to everyone but Heroes get to input ideas and ask for things directly? I sort of think of this tier as my "inner circle" to discuss everything with and share thoughts.
I'm still super new to all of this so I'd love any advice that you guys have from other patreons you follow or other commission sites. Do they do something different? What works and what doesn't? Any suggestions are welcome!
