it varies. for this one, i started with a reference shot that gave me the basic camera angle and perspective of the image (looking nothing like the final pic in this case). then i drew a rough sketch( https://i.imgur.com/qQPalx4.jpeg ), and from there it was just a matter of refining, reworking stuff i didn't like, looking up more reference for areas i struggled with as i went.
