To answer the question at the front of this: There are multiple changes around how we work and what we work on that have lowered general risk of infection. The fact that you're going to get cut by a piece of paper or a paring knife instead of, say, a scythe or a pitchfork tine is a part of it. Germ theory is another part, and the degree of cleanliness considered valuable to surgeons was a debated (!) topic until germ theory became accepted science. Another thing to consider is that overall diet and health have an effect on your immune system, and the Little Ice Age caused some of the worst famines in European history (can't speak to the rest of the world, but probably not great there either). However, it's also important to remember that not every injury was a death sentence. We have skeletal and textual evidence of combatants surviving multiple cut and puncture wounds, and patients surviving operations as invasive as trepanation.
