I still hate this episode. It hurt so much watching Dean die and knowing what that meant. I was seriously pissed at Kripke and everyone responsible with this storyline. I had held out hope that he wouldn't die. It's still really hard to watch this episode. I was mourning season 1 Dean during season 2, and then season 2 Dean during season 3. And now . . .
My heart still hurts for Dean going to hell.
I don't think there's any rhyme or reason to what Lilith or Ruby are doing in this episode. They're playing with Sam. I think Lilith would have been happy if Sam died, since she seemed unhappy that she had to die. And Ruby is totally playing with Sam. They both wanted Dean in Hell, because John wasn't breaking and they needed that to start breaking the seals. So Ruby had to get Sam on board with her plan. Without Dean, she had a better chance of it.

I think of both episodes 14 and 15 as hybrids, because while they are MOTW the heart of the quest for Dean (ep 14) and Sam (ep 15) is to save Dean from hell.
Dean is really desperate in ep 14, willing to believe anything. Then Episode 15 really shows us how unhinged Sam gets trying to save Dean. Terminator Sam in Mystery Spot is one thing. In ep 15, Sam being willing to turn Dean into a living Frankenstein-type monster is truly next level, considering it means harvesting living body parts on the regular for the rest of eternity. Yikes.

I love this finale. And as much as I hate how much Sam is toyed with, I can't help but respect the demons for just how deep the con was. the way I always interpreted the demon plan was, it all started at the end of season 2. The special children were made specifically to have people strong enough to kill Sam. He was always going to die at that abandoned town. Yes they originally wanted John to break the seal, and then had to go with Dean when that didn't work, but killing Sam was as much about getting Dean's soul as it was starting the process of severing the bond between the brothers. Sam was brought back, and all Azazel had to do was make Dean question how much of Sam actually came back. Then Lilith and Ruby could strike, playing good cop/bad cop with Sam. Ruby set up Lilith to be this big horrible monster for the boys to take down, and set out to making the boys trust her, being as helpful as possible while Lilith made it seem like it was her life's work to kill and torture them. Then this happens. Sam watches his brother die. He is utterly broken, and Ruby comes in and takes on Dean's role, of supporting and protecting and caring Sam back into being a hunter, a soldier. She forges a bond with him and sets him on the path to kill Lilith. I fully believe Lilith and Ruby intended to break every seal from the beginning. Lilith was ready to die for that, and Ruby was to. But they couldn't ever let Sam catch onto that. he needed to believe Lilith didn't want this. That Lilith was looking for ways out, that she maybe wasn't so on board with freeing Lucifer, because that kept Sam hot on her trail. Later when we see Lilith try to make a deal with him, I fully believe she was just sensing that Dean was starting to get through to him, and that he was faltering on his decision, so she went out and gave him more of an excuse to want her dead. Lilith has been playing with Sam since he was a baby. Using Azazel, and Brady, and Ruby to keep things moving the way she wanted. And Sam I think could have seen through it, with Dean's help. I fully believe that together they could have figured it out. But that is why the angels came in. Season 4 Sam was at his lowest point, his most vulnerable and it needed delicate handling from Ruby. Dean would never be as able to be manipulated by demons as Sam, so Lilith needed Michael's help. The angels could get into Dean's head. They could make him doubt, make him worry, and spin him out so that he does what any older brother would- try anything to bring his little brother to heel. The levels of manipulation on both boys is heartbreaking, and tragic and freaking brilliant on both ends. It's what makes season 4 so hard to watch for me- the apocalypse is both of their faults, and neither of their faults, at the same time, and they didn't stand a chance. Not with Heaven and Hell against them, and not already knowing what they inevitably learn from this entire experience

Demons are sneaky and we learn later that Lilith really doesn't want to die so I believe none of the white eyed demons involved with breaking the seals can hurt Sam with the light thing as a precaution just in case they get any ideas to stop him before killing Lilith. He is Lucy vessel so he must be protected.
Lilith for a year was after Sam to kill him most likely because she is not really ok with breaking seals to lead to her death . She was trying to get out of her deal with Micheal. (So it would be a nice parallel to Dean) So she was just having fun until she saw him be immune. She also knows she has to start it by sending Dean to hell. So her and Ruby's plan is to get them there and kill Sam if possible to save herself from dying. Dean was already set to die regardless of what she does and she specifically has his contract instead of Crowley for this reason.
Ruby got out of the seal and it is never explained but Lilith definitely got her out somehow and Ruby allowed her to take her body to trick Sam and Dean.
