A Date with Death Voiced Name Form

We're focussing on a lot of character customization, and the game will feature voiced calls. During those calls, we'll have some opportunites for the character's name to be said by the love interest, if their name is a 'voiced name'.
That's where this form comes in - for the next few weeks, Patrons will have a chance to submit up to two names they would like to be included as 'voiced names' within the game.
We reserve the right to exclude any that are offensive in any way, though you're welcome to put in 'real' names, fantasy names, anything you would like within reason.
