The gunshot certainly happened. I see it as being fired by Theo. It was his exact promise to put a bullet into Lucas' head if he ever touched Klara again. We have just seen Lucas carry Klara over the patterned floor. I believe the missing of the shot was intentional but also a metaphor for truth not hitting the mark.

I think it happened for sure and is a reminder that what happened doesnt just go away because he is welcomed back into the community. I think if it had to be anyone, it was probably Carla's brother.
