Xamin knows that Elise's powers are killing her slowly, and he did mention it to Elise that he will FIX IT. He says that as soon as they remove the evil source she'll be okay. And he's so hyper-focus on FIXING it that he doesn't even give her time during her struggle, always staying distant. In this scene, Xamin realizes that he can't save her after everything. And that's when he realizes how much she means to him, but it's too late now. Elise was already prepared for worse, so she just knows this is her last time around him. She doesn't blame him for anything, he was just trying his best after all.
It's the first time he hugs her.
