I think the time ticking was just metaphorical. Remember (an episode or two ago maybe?) Mike said Walt was a ticking time bomb. Also, with Skylar saying she's just waiting for the cancer to come back. I think it's just to remind us he only has so much time. Pretty cool the composer also thought to include it in the outro song tho.

It's just that one life-threatening illness made him no longer inhibit his narcissism. This was the point where people think this is where Walt first diminished Skyler, but the reality was that he had her by the metaphorical balls at that point. She couldn't go to the police, she couldn't leave him, he trapped her ages ago.
