Yeah, it was really sickening and vile to watch that scene, knowing that she had gone through that. now that I think about it, it wouldn't have been common knowledge for the writers or people on the show because it wasn't until January 2020 at a Con called UnityDays which is a convention surrounding the 100 and she with Bob when she announced that she was pregnant but then suffered the miscarriage. She was supposed to direct an episode of the 100, just like Bob had directed 6x11 and she was supposed to direct 7x07 but had to give it up to someone else as she was pregnant but then had the miscarriage. Bob got a tattoo and its a penguin tattoo of a father penguin and a baby penguin which he posted a picture of on his Instagram with the caption "I got this tattoo at a turning point in my life, things don't always work out as planned, but the lessons learnt have made me a stronger and better human."
So I believe it was known to people on the show that Eliza was pregnant and that's why she had to step back from directing but didn't know about the miscarriage.
Though on a happy note, Eliza and Bob announced on February that Eliza is pregnant, they are going to be parents and the baby is on the way. they even had a baby shower and the actors that played Atom, Roan and Monty came along with other friends too which was so cute.

Noooo wayyy. I had no idea that Eliza suffered a miscarriage... dude, that's so messed up that she had to act a scene SO personal after going through something as unimaginable as that. I'm actually mad, for real. was this common knowledge?

i absolutely despise that scene with Clarke and Madi, it's so heartbreaking and its worse behind the scenes because the actress who plays Clarke, Eliza Taylor suffered a miscarriage whilst filming the last season and even if the writers knew about the miscarriage or not, it's still fucked up to get the lead actress of the show to come in and act like their losing their onscreen child when they've just experienced the loss of a child in their personal life. it's just sad and sickening to think about especially when Clarke says "My baby." the way she does like its just absolutely heartbreaking to witness and hear.
