But wait, doesn't she survive to finish keiichi off in the hospital bed in the final scene of arc 2. I can't remember if that was ever explained.

Only this time told from the killer's perspective, rather than the perspective of the hero (Keiichi). It's the one where we get all the answers to the unanswered questions from the second arc. Why was the body of Mion Sonozaki found at the bottom of the well? When Mion was the one who stabbed Keiichi? The answer is simple, it was Shion impersonating her sister. She set up this whole ruse to discredit and destroy the Sonozaki family, as well as its heir. Her sister Mion, who was more or less a pillar of the community. Of course this being Higurashi, she goes batshit insane while accomplishing this plan. She also doesn't get away with it scot-free, karma eventually catches up with her for all the evil she's done. In this case she falls to her death, when her plan has finally come to fruition and it appears she's gotten away with everything.
