The one who is dead comes from the ranks of the calamity's victims from previous years. Reiko was killed a year and a half ago, as homeroom teacher of Class 3. The calamity was particularly insidious this year. Think about it, it's easy to create an extra student who's been dead for possibly decades. All you have to do is add them to the class list and have them show up. With Reiko you have to alter the memories of a bunch of people who went to her funeral a year and a half ago. Also it suddenly makes sense why Izumi was so hell-bent on protecting everyone. Her cousin who was really more like a brother to her, died the same year Reiko did. She had a personal stake in it. Of course her desire to protect everyone, was clouded by her crush on Kouichi, her jealousy towards Mei's closeness to Kouichi and later her grief of having her friend Takako die. Someone who's been her friend for who knows how long. She's honestly a tragic character in that sense.
