Damon is right--that cure conversation on the island was completely different. Elena was still affected by the sire bond at that point and Damon was still very insecure about her feelings for him. He was protecting himself. After all, all he heard from Katherine since her return and from Elena up until recently was, "I love Stefan and it will always be Stefan." He couldn't bring himself to believe that Elena actually loved him. Now he does.
