Illyasviel’s backstory is very twisted. As was explained in this episode, it seems her father, Kiritsugu, killed her mother, Irisviel, and then started a new family by adopting Shirou (so yes Illyasviel is technically Shirou’s sister). We don’t know why this happened at this point but it is the reason Illyasviel seems to have a vendetta against Shirou. Illyasviel’s backstory is definitely elaborated on in Fate/Zero (the prequel)
