Basically, the late emperor feared women because his mother basically browbeat him into his position and he feared adult women due to her overbearing nature and control over him. He ended up only being to talk to or even be near young girls which unfortunately caused his attraction to them. Because that was one of the only control he had. After all he was universally disliked and deemed incompetent. As complex and controversial as he was, even Anshi pitied him. So yes, Anshi knows about the switch and consents to it. Unfortunately, she could not love a child born from her hatred and act towards the emperor. However, it’s goes without saying she didn’t want the baby to die.
