While I don't know if this is accurate (anime only), I would guess that Satella is mad that Subaru told Echidna about RbD — a secret only meant to Subaru and Satella — not to mention he told Echidna so many times and about everything that's happened. I think the dark hand we see flash when Subaru first tells Echidna about RbD is either Subaru expecting the pain that doesn't come, or it's a sign that Satella was reacting to Subaru telling Echidna. I don't know if Satella is actually crazy, but we've been told in season 1 that she consumed the other witches — the other witches are already a crazy in their own way due to their own power, so I can't imagine Satella's mental health/mind is stable given all 7 sins are within her.
