Every single thing that has gone wrong since episode one Joel has taken as a personal failure. He failed Sarah, he failed Tess, he got to Bill's too late, he lost the car in Kansas city by falling for the trap, he didn't hear Henry and Sam coming, he let himself warm up to Henry and Sam and both failed to protect them, and failed to protect Ellie from him. He's had this constant lingering fear that he's going to fail to get to Tommy in time It's loss after loss after loss and he takes all of these things, whether in his control or not, as his own fault for personally falling to protect people. That's why it all comes to a head so violently that it manifests as physical symptoms. He can't handle it anymore, and the more he cares for Ellie the more terrified he becomes of failing her again. It's heartbreaking.
