I think they said in a podcast that they were going to end episode 1 after Sarah died but said it would be too much of a down note. And the song at the end is a different version of the song at the end of episode 1

The thing about Joel's fears and failures is that it isn't just about Sarah either. Every single thing that has gone wrong since episode one Joel has taken as a personal failure. He failed Sarah, he failed Tess, he got to Bill's too late, he lost the car in Kansas city by falling for the trap, he didn't hear Henry and Sam coming, he let himself warm up to Henry and Sam and both failed to protect them, and failed to protect Ellie from him. He's had this constant lingering fear that he's going to fail to get to Tommy in time It's loss after loss after loss and he takes all of these things, whether in his control or not, as his own fault for personally falling to protect people. That's why it all comes to a head so violently that it manifests as physical symptoms. He can't handle it anymore, and the more he cares for Ellie the more terrified he becomes of failing her again. It's heartbreaking.

Man, that whole moment of Hman and SeeOhKnee just theorizing and speculating while Ruff and Boom quietly drink was so satisfying and hilarious.
