The ghost in the end with Theo and Hugh was the mother. If you remember Theo's vision when she touched her mother's hand, she had a huge open injury in her head, just like the ghost at the end and the one in the cemetery pulling Luke's arm to the grave. This episode does not have a lot of things happening in comparison to the others, but it adds so much detail to the story, like Dudley's backstory that is so freaking sad, more information about the Hills and the father's inside thoughts and how broken all their dynamic is.
